As discussed I need some more fields in the form for English site.

Please add two address lines. So it will be address line 1-3.

Make the zip code wider and but the city field under the zip - not in line as it is now.

Then insert a drop down for country selecting. I need to ab able to control what countries are in the drop down.

Also please add a drop drop down for states if country is USA.
